Once again, thank you to all of you who have participated in our Sculptra Fundraising Program! As a way of saying thank you for all of your support, we are now beginning the Conant Foundation Free Sculptra Treatment Program for those with facial lipodystrophy syndrome (FLS).
The Conant Foundation is sponsoring eight free treatments of Sculptra per month. (These typically cost $400 per treatment.) Patients who qualify for free Sculptra treatment will have their names placed into a lottery. Those who are selected will be treated with Sculptra (1-6 times) over a period of 6 months until it is the judgment of the provider that adequate correction/restoration has been made.
Criteria
- Patient must qualify for Dermik’s Sculptra patient access program.
- Patient must not be covered by Kaiser Permanente or Brown and Toland.
- Patient must not have Veterans Administration health benefits.
- Patient must only be supported by Social Security, Medicare, Medi-Cal and/or ADAP.
- Patient must have an income of less than $1300.00 per month. Proof of income can be either a social security statement or an IRS tax return.
- Patient has not detected any changes in facial lipodystrophy in the past six months.
Those who qualify must complete Dermik’s Sculptra Access Program Form, our patient registration form, and provide proof of income.
